Payment Queries
Insurance (Assignment)
If you have dental benefits with assignment, we can bill your provider directly. When arriving to the office, please have your policy number, coverage plan, and if applicable, student ID, ready so that the process is as seamless as possible. Please keep in mind that any fees not covered by the plan (deductible, partial coverage fees, differences, co-pay) are due on the day of the procedure.
Insurance (Non-Assignment)
If you have dental benefits with no assignment, you will have to pay in full for the procedure and request reimbursement from your provider. Our office will be able to aid with any paperwork required once payment is made on the day of procedure. Please have your policy number, coverage plan, and if applicable, student ID, ready so that the process is as seamless as possible.
ODSP and HealthySmiles
Our office accepts both ODSP and HealthySmiles patients. Please ensure you have your benefits card and information for the applicable month. It is your responsibility to pay for the procedure if your information and/or benefits card are invalid and to also sign a claim form for treatment.
Uninsured
For all our patients without dental benefits of any kind, we accept VISA, Mastercard, and cash for payment of procedures due in full the day-of.
